HeartToHeart launches a beta test of “MY STATUS AND VACCINATIONS” survey and personal report.
Please partner with us in an exciting opportunity to improve the understanding of how this period in public health is affecting your household and community. As one of the 100 beta testers, your comments and suggestions will help develop better tools to monitor and determine the uptake of vaccinations.
With your help, our goal is to generate a status report which you can share in informal and formal settings. This may include: applying for a job, travel, school functions, sports, medical visits, and other activities.
You can contribute by completing the survey and follow-ups regularly and providing us feedback on the usefulness of the survey and the functions of the application.
At any point in time, you can generate a report showing your responses to your symptoms and vaccination status. You can withdraw from the program at any time for any reason.
No personally identifiable information will be shared except where required by law.
NOTE: When you sign in the first time, you will see a baseline survey which should take less than 5 minutes to complete. The sequence will be: 1.) account creation screen; 2.) registration screen; 3.) baseline survey; 4.) additional personal information (optional); 5.) additional contact information (optional); 6.) home screen. A white box on the home screen indicates that you have previous responses that you can view by selecting that box. You may also generate a personal report by select “Report” from the menu (the three-bar icon in the upper left). You may download this report in a document format or spreadsheet format for printing.
